My Health Online supports adults living with chronic health conditions such as type 2 diabetes, cardiovascular disease and hypertension, overweight and obesity, metabolic syndrome, and fatty liver disease.
It’s designed for people who already understand what they should be doing, but find it difficult to turn advice into consistent action once appointments end or life gets busy. We provide structured, practical support that helps healthy changes fit into everyday routines, not short-term fixes.

My Health Online provides GPs with a structured, coordinated model of care for patients with chronic conditions. Patients receive personalised dietitian support, access to a comprehensive Lifestyle Program, and ongoing communication throughout their plan, helping them stay engaged between appointments and translate advice into consistent action.
Progress updates are shared with the referring GP, supporting continuity of care.

Personalised support to help you take control of your health

With a GP referral, you can access bulk billed telehealth dietitian appointments and a structured Lifestyle Program designed to support real, lasting change. Your dietitian works with you to set clear goals, build practical habits, and guide you step by step, so you can make confident changes that fit into your everyday life.
How it works

A simple process designed around your care

Our care pathway combines personalised telehealth appointments with a structured Lifestyle Program, so there is support at every stage. Each step builds understanding, confidence, and practical skills, helping people make steady changes that last.

1. GP referral

Your GP prepares a Chronic Disease Management Plan and refers you to My Health Online. We then contact you to book your first appointment and get you started.

2. Your first appointment and program access

Your dietitian completes a full assessment and works with you to set clear, practical goals. You are then given access to the self paced Lifestyle Program, so you can begin building skills and making changes between appointments.

3. Ongoing support

Appointments continue every 3 to 4 weeks, giving you regular support and accountability. You work through the Lifestyle Program between sessions, while your dietitian helps you review progress, personalise your plan, adjust goals, and stay on track.

4. Completion and next steps

At the end of your plan, you receive a progress summary outlining your achievements and next steps. Your GP is kept informed throughout your care, supporting continuity and coordinated management. If further support is needed, you have the option to continue privately.
