A coordinated, dietitian-led Chronic Disease Management service designed to support long term lifestyle change, improve patient engagement between appointments, and maintain clear communication with referrers.
Who My Health Online is for
Supporting patients who need more than one-off allied health appointments

My Health Online supports adults living with chronic conditions who benefit from structured, ongoing lifestyle support delivered alongside their Chronic Disease Management Plan.
This service is particularly suitable for patients who:
Common referral presentations include, but are not limited to:
How Chronic Disease Management fits into the model
Using limited Medicare-funded sessions more effectively
Under Medicare, patients are eligible for up to five subsidised allied health sessions per calendar year through a Chronic Disease Management Plan. These sessions are shared across all allied health disciplines.
While CDM provides valuable access to care, limitations often include:
My Health Online is designed to work within this framework by combining bulk-billed dietitian appointments with a comprehensive Lifestyle Program that supports patients between sessions. This integrated approach helps patients stay engaged, apply strategies in daily life, and make better use of their limited CDM sessions, without additional program costs.
For GPs, this means referrals that support continuity of care while reducing the need for repeated short-term interventions.

Why this approach works
A smarter way to use Chronic Disease Management Plans
Chronic Disease Management plans offer valuable access to allied health, but the model often leads to fragmented, short-term input across multiple providers.
My Health Online is designed to work within this system while addressing its key limitations. By combining dietitian-led telehealth care with an integrated, multidisciplinary Lifestyle Program, we provide coordinated, structured support that continues between appointments, not just during them.
This coordinated model enables:
More effective use of limited allied health sessions
Care builds across appointments instead of being spread thin across providers.
Consistent patient engagement between reviews
Structured guidance helps patients stay on track when motivation fluctuates.
Unified, evidence-based lifestyle messaging
Reduces confusion from multiple disconnected sources of advice.
Observable progress over time
Goals, strategies and learning are reinforced across sessions and the Lifestyle Program.
This approach shifts care from isolated advice to structured behaviour change support, helping patients develop the understanding, skills and confidence needed for sustainable health improvement.

The Lifestyle Program
Integrated multidisciplinary Lifestyle Program
All patients referred to My Health Online receive access to an integrated online Lifestyle Program that works alongside dietitian appointments to reinforce learning, support behaviour change and maintain progress between sessions. The program has been developed with clinical input across dietetics, psychology, exercise and specialist medical care, ensuring consistent, evidence-based lifestyle guidance that complements primary care management and supports chronic disease treatment.
