This service is particularly suitable for patients who:
  • Have a chronic condition lasting six months or longer
  • Are eligible for a Chronic Disease Management Plan
  • Would benefit from dietitian-led care supported by education and behaviour-change strategies
  • Have struggled to maintain progress with fragmented or short-term allied health input

Referral process

Simple steps to get patients started

Referring to My Health Online is straightforward and designed to fit easily into existing Chronic Disease Management workflows. Once a referral is received, our team manages patient contact, appointment scheduling and program access, keeping you informed as care progresses.
Patients who are not eligible for a CDMP, or who have already used their sessions, can still be referred and seen privately.

Prepare a Chronic Disease Management Plan

Confirm eligibility and include key clinical information so lifestyle care can be delivered in a way that supports your treatment priorities and medical management.

Step 2.

Refer the patient to My Health Online

Submit the referral via the secure online referral form on our website or send it directly via email. Both CDMP and private referrals are accepted.

Step 3.

Patient contact and onboarding

After receiving the referral, our team contacts the patient to schedule their first telehealth appointment. Once booked, they are given access to the My Health Online Lifestyle Program to begin.

Step 4.

Ongoing communication

You receive structured updates at key points in care, including progress summaries and recommendations, to support your ongoing clinical management and decision-making.
