About My Health Online

Understanding the service

My Health Online is a dietitian led telehealth service for adults living with chronic health conditions. Care is delivered through Medicare funded appointments under a Chronic Disease Management Plan and is supported by an integrated Lifestyle Program that provides structured education and behaviour change support between sessions.

Many allied health services rely on individual appointments alone. My Health Online combines one to one consultations with a structured Lifestyle Program that supports learning, habit building and reinforcement between sessions.
This coordinated model helps patients maintain momentum between appointments and allows limited CDM sessions to be used more effectively for assessment, problem solving and clinical review.
No. My Health Online works alongside your GP and does not replace your medical care. Your GP remains responsible for diagnosis, medical management and overall treatment planning.
We provide dietitian led lifestyle care and structured education, with updates to your GP to support coordinated, informed care.
My Health Online supports adults living with chronic conditions where lifestyle change plays an important role in management. This includes:
diabetes and insulin resistance, obesity and weight related health conditions, fatty liver disease, PCOS, hypertension, cardiovascular risk factors and long term mental health conditions such as depression.
Telehealth works well for many people, but suitability depends on individual needs. Your GP can help determine whether telehealth dietitian care is appropriate for you.
My Health Online is designed for adults managing stable, long-term chronic health conditions where lifestyle support plays an important role. It is not intended to replace specialist or intensive services.
The service is not appropriate for people who:
  • Require treatment for eating disorders or complex disordered eating
  • Are seeking bariatric surgery preparation or post-operative bariatric care
  • Have active gastrointestinal conditions requiring specialised therapeutic diets, such as inflammatory bowel disease or complex IBS management
  • Have unstable or acute medical conditions that require close in-person or specialist medical supervision
  • Need paediatric or adolescent care
  • Are seeking highly specialised medical nutrition therapy outside the scope of general chronic disease lifestyle management
In these situations, your GP may refer you to a more specialised service that better meets your needs.
Chronic Disease Management (CDM)

How Medicare-funded care works

A Chronic Disease Management Plan is prepared by a GP for patients with a condition that has lasted, or is expected to last, six months or longer. It allows access to Medicare-subsidised allied health services. Learn more about the CDM plan here.
Medicare provides rebates for up to five allied health sessions per calendar year under a CDM Plan. These sessions are shared across disciplines such as dietetics, physiotherapy and exercise physiology.
Yes. Patients can choose how they use their available allied health sessions. My Health Online works within the CDMP framework alongside your ongoing GP care, supporting patients to make effective use of their sessions as part of their overall management.
Yes. Eligible dietitian appointments are fully bulk billed under a Chronic Disease Management Plan, with no out-of-pocket cost.
Once your CDMP sessions are completed, you may choose to continue your progress with additional private appointments if appropriate. This can be discussed with your dietitian and GP as part of your ongoing care plan.
At present, My Health Online cannot be accessed under Department of Veterans’ Affairs (DVA) funding arrangements.
DVA policy does not support dietitian services delivered exclusively via telehealth, which means our fully online model does not meet current DVA eligibility requirements.
This limitation relates to DVA funding rules rather than the type or quality of care provided. Veterans are still welcome to access My Health Online privately if appropriate for their needs.
The Lifestyle Program

Support between appointments

The Lifestyle Program is a structured, self-paced online program that supports patients between and alongside dietitian appointments. It reinforces key skills across nutrition, movement, sleep, stress, and long-term behaviour change so patients can build momentum between sessions.

The program was developed with input from clinicians across dietetics, psychology, exercise physiology and specialist medical fields. It is designed to complement dietitian-led lifestyle care and support GP-led care plans through consistent, evidence-based lifestyle education and practical behaviour-change tools.

There are no additional program fees for patients referred under a Chronic Disease Management Plan the lifestyle program is included as part of structured My Health Online care.
For patients accessing care privately, program access may be incorporated into their appointment package.
Access is provided after your referral is received and your first appointment is scheduled. You can use the program on your phone, tablet or computer at your own pace, with key modules aligned to the focus of your dietitian sessions.

Getting started with My Health Online

For referrers

Prepare a Chronic Disease Management Plan (for Medicare-funded care) or a referral letter (for private patients), and send the referral to My Health Online via our secure online referral form or by email.
Yes. Referrers receive updates at key points in care, including summaries of goals, progress and recommendations, supporting continuity of care and your ongoing clinical management.
Yes. Referrers are welcome to contact us with questions about referrals or patient suitability. We are happy to discuss how the program may support your patient alongside your clinical management.

For patients

No. You can access My Health Online as a private patient without a referral.
However, if you would like to access bulk-billed dietitian appointments under Medicare, you will need a GP referral through a Chronic Disease Management Plan.
Yes. If your GP has already prepared a Chronic Disease Management referral or if you are accessing care as a private client you can book your first appointment online here.
Once a referral is received, our team contacts patients as soon as possible to offer the next available appointment with their clinician and provide access to the Lifestyle Program so care can get started promptly.
