My Health Online delivers an integrated model of chronic disease care that combines fully bulk-billed dietitian telehealth with a structured, multidisciplinary Lifestyle Program.
Rather than offering isolated appointments or one-off advice, this program is designed as a coordinated end-to-end service. Patients receive structured support across nutrition, movement, sleep and mental wellbeing, with clear goals, follow-up and communication with their GP.
This model helps patients use their Chronic Disease Management sessions in a way that supports real, sustainable long-term change.
How Medicare-funded allied health support works

Understanding the Chronic Disease Management Plan (CDMP)

A Chronic Disease Management Plan (CDMP) is a Medicare initiative that helps people with a chronic medical condition access subsidised allied health services.
To be eligible, a person must:
  • Have a chronic condition lasting six months or longer
  • Have a GP who prepares a CDMP
  • Be referred to allied health providers, such as dietitians, physiotherapists or psychologists
Under a CDMP, Medicare provides rebates for up to five allied health sessions per calendar year. These sessions are shared across all allied health disciplines.
These sessions are commonly used for:
  • Dietitian appointments
  • Exercise physiology or physiotherapy
  • Podiatry
  • Psychology or other allied health services

Where CDMP support often falls short

While CDMP plans are valuable, they come with practical limitations that can make meaningful, long-term change difficult without additional structure and continuity.
Common challenges include:
  • Only five sessions per year, regardless of how complex the condition is
  • Sessions are shared across all allied health providers, which can limit continuity
  • Limited appointment numbers often mean care stays surface-level rather than deeply personalised
  • Appointments are frequently spaced far apart or focused on short-term issues
  • Little structured support between sessions
For many people, this can lead to fragmented care, repeated or generic advice, and difficulty maintaining progress once appointments end. Even with great clinicians, it is hard to achieve meaningful change when support is limited to just one or two sessions.

How My Health Online supports better outcomes

My Health Online was designed to work within the CDMP framework while overcoming its practical limitations. Dietitian-led telehealth appointments are bulk billed for eligible patients and are supported by a structured, multidisciplinary Lifestyle Program included as part of the service. This means patients receive coordinated care, ongoing education, and practical tools that extend beyond individual appointments.

How the program works

A clear, coordinated pathway from referral to real progress

Managing a chronic condition requires more than isolated appointments.
The My Health Online program follows a structured, step-by-step pathway that combines GP referral, bulk-billed dietitian-led telehealth care, and a comprehensive Lifestyle Program developed by a multidisciplinary team.
Each part of the journey builds on the last, so patients receive coordinated support, not one-off advice. All components are included as part of the Chronic Disease Management Plan referral, so eligible patients do not pay additional program fees.
Step 1.

GP referral and Chronic Disease Management Plan

Your GP prepares a Chronic Disease Management Plan and refers you to My Health Online. This referral allows you to access Medicare-subsidised allied health services, including dietitian appointments.

Step 2.

Appointment booking and program access

Once we receive your referral, our team contacts you to book your first telehealth appointment and provides access to the My Health Online Lifestyle Program. The program is included as part of your CDMP referral, with no additional program fees.

Step 3.

First dietitian telehealth appointment

In your initial appointment, your dietitian reviews your medical history, current lifestyle and health goals. Together, you identify priorities and develop a practical plan tailored to your needs. You are also guided on how to use the Lifestyle Program between appointments. This ensures your care plan extends beyond the appointment itself.

Step 4.

Ongoing lifestyle support between sessions

Between appointments, you work through the structured online Lifestyle Program at your own pace. This program goes beyond reinforcing session content, it provides deeper education, practical tools, and multidisciplinary guidance across nutrition, movement, sleep, stress, and behaviour change.

It introduces new concepts, builds understanding, and supports you to develop the skills needed for lasting lifestyle improvements. This ensures care continues between telehealth sessions, not just during them.

Step 5.

Follow-up appointments

Follow-up dietitian appointments are typically scheduled every three to four weeks, depending on your CDMP allocation and needs. These sessions focus on reviewing progress, refining goals, addressing barriers, and building on the work completed between appointments.

This is where care becomes highly personalised, supporting accountability, problem-solving real-life challenges, and adjusting the plan so it works for you. Eligible appointments are fully bulk billed.

Step 6.

Communication with your GP

Your GP receives clear updates at key stages, including progress summaries and recommendations. This supports continuity of care, keeps your broader health team informed, and helps ensure decisions are made with a full picture of your progress.

Step 7.

Ongoing care options

Once your available CDMP sessions are completed, we review your progress and goals together. If ongoing support would be helpful, you may choose to continue with private appointments tailored to your needs.

We are also developing additional structured follow-up programs and support options within the online platform, allowing patients to continue building skills and maintaining progress over time.

The program has been developed with input from dietetics, psychology, exercise physiology, and medical specialist expertise in endocrinology, ensuring a whole-of-lifestyle and medically informed approach to chronic disease management. It introduces concepts, skills, and practical strategies that go beyond what can be covered in individual appointments alone.
